Korean Small Face Treatment: Non-Surgical Face Slimming in Seoul
Korean small face treatment is a tailored combination of non-surgical procedures, such as jaw Botox, contour injections, lifting and filler, that makes the face look smaller and more V-shaped by treating whatever is creating its width. At Hommage Clinic in Gangnam, Seoul, the plan starts with a facial analysis of muscle, fat, skin and proportions, and is reviewed by Dr. Kim Hyunbin.
Quick answer
Korean small face treatment is a non-surgical plan that makes the face look slimmer by treating whatever is creating its width: jaw muscle with Botox, local fat with contour injections, sagging with HIFU, RF or threads, and a short chin with filler. At Hommage Women's Clinic in Gangnam, Seoul, Dr. Kim Hyunbin starts with a facial analysis. Visits take about 15–60 minutes, jaw Botox slims over 2–6 weeks and lasts about 4–6 months, and lifting results last about a year.
Key takeaways
- Face width comes from four layers: bone, muscle, fat and skin. Only the last three respond to non-surgical treatment.
- The right combination depends on the cause; a clench test and a pinch test are simple first clues.
- Results are gradual and natural rather than dramatic, and most components need maintenance.
- If bone is the main cause, an honest clinic will say so before you book anything.

Small face treatment may suit you if you have
- A square or wide lower face from strong jaw muscles
- Local fullness under the chin, along the jaw or in the lower cheeks
- Early sagging that blurs the jawline and widens the lower face
- A short or recessed chin that makes the face look rounder
Wait or consider other options if you
- Are pregnant or breastfeeding (most injectables are postponed)
- Have a neuromuscular condition such as myasthenia gravis (Botox is avoided)
- Want to change bone width, which only surgery can alter
- Have an active skin infection or a bleeding disorder
What is Korean small face treatment?
Korean small face treatment (sometimes called 소두 or “small face” care) is not one procedure but a plan. A face can look wide or heavy for several different reasons, and each reason responds to a different treatment. The aim is a slimmer, softer outline, often described as a V-line, while keeping your face looking like you.
Four layers shape the width of the face: bone, muscle, fat and skin. Proportions matter too: a short chin or flat forehead can make the same face look rounder. Non-surgical treatments can change muscle bulk, local fat, skin firmness and some proportions. They cannot change bone.
What makes a face look wide, and what treats it?
The right treatment depends on the cause of the width, so the first step is working out which layer is responsible.
- Strong jaw muscles (masseters): a square lower face, often in people who clench or grind. Jawline Botox relaxes and slims the muscle over several weeks.
- Local fat: fullness under the chin, along the jaw or in the lower cheeks. 3D contour injections and other fat-dissolving injections reduce small, defined fat pockets.
- Sagging tissue: early jowls and a blurred jaw edge widen the lower face. Lifting with HIFU (Ulthera) , RF devices or threads repositions and firms tissue.
- Proportions: a small or recessed chin can be lengthened with chin filler , which makes the lower face look narrower.
- Bone: a wide jaw angle or prominent cheekbones come from bone structure, which non-surgical treatment can soften visually but not reduce.
How is a small face plan built?
Most plans combine two to four treatments, ordered so each one works on the right layer. A typical sequence looks like this:
- Analysis: the doctor assesses the face from the front and side, checks jaw muscle bulk while you clench, and feels for fat and skin laxity.
- Muscle first: if the masseters are large, Botox is often given early because its effect takes 2–6 weeks to build.
- Fat and lift: contour injections reduce local fat over a series of sessions, while HIFU, RF or ONDA energy treatment firms the skin around it.
- Balance: filler or a thread lift is added when chin projection or sagging is still limiting the outline.
Many treatments can be combined in one visit, which suits patients on a short stay in Seoul. Others, such as repeated contour injections, need visits a few weeks apart.
How long do small face results last?
Each component has its own timeline, so results build in stages and need different maintenance.
- Masseter Botox: slimming appears over 2–6 weeks and typically lasts about 4–6 months. With regular treatment, the muscle often stays smaller for longer.
- Contour and fat-dissolving injections: change appears gradually over several weeks after each session, and usually 2–4 or more sessions are needed. Fat cells that are destroyed do not come back, though remaining cells can enlarge with weight gain.
- HIFU and RF lifting: collagen remodeling builds the effect over 2–6 months; results typically last around a year.
- Threads and filler: threads usually last about 6–18 months; chin filler often lasts about 12–18 months, depending on the product.
Risks and possible side effects
Every component carries its own risks, and the doctor will go through those that apply to your plan.
- Botox in the jaw: temporary chewing fatigue, mild tenderness or bruising, and, uncommonly, an uneven smile if the toxin spreads to nearby muscles. Over-treatment can make the cheeks look slightly hollow.
- Contour injections: swelling, bruising, tenderness and temporary numbness are common for several days. Lumps, nodules or, rarely, nerve irritation or skin injury can occur.
- Energy devices: redness and swelling; rarely burns, blisters or temporary numbness.
- Threads and filler: bruising, asymmetry, palpable threads or lumps, infection and, rarely with filler, blocked blood vessels, which needs urgent treatment.
Contact the clinic straight away if you notice skin turning pale, gray or dusky after an injection, worsening pain, fever, difficulty swallowing, or a change in your smile or vision.
Non-surgical vs surgical facial contouring: which is right for you?
Non-surgical small face treatment changes soft tissue; facial contouring surgery changes bone. Surgery, such as mandibular angle reduction or zygoma reduction, is performed by plastic or oral-maxillofacial surgeons under general anesthesia, with weeks of recovery and permanent structural results. Hommage Clinic does not perform bone surgery. If your width comes mainly from bone, the doctor will tell you so and explain what non-surgical treatment can and cannot change. Our guides to cheekbone reduction and buccal fat reduction explain those surgeries and their non-surgical alternatives.

How it works
How does small face treatment work? The four layers of face width
Imagine a cross-section through the lower face at the jaw angle. From the inside out there is bone, the masseter muscle that you use to chew, a layer of fat, and the skin and its support tissue. Any of these can add width, and most faces have more than one contributor. A small face plan works out which layers matter most for you, then chooses a treatment for each.
This is why two people asking for the same thing, a slimmer face, can leave with very different plans. One may need only jaw Botox; another may need a lifting treatment and a little chin filler, with no Botox at all.
Self-check
Muscle, fat or sagging? Simple clues before your consultation
A doctor's assessment is the only reliable way to tell, but a few simple checks in the mirror can help you understand what you will be asked about. These are clues, not a diagnosis.
Likely muscle
Place fingers at the jaw angle and clench. A firm bulge that pushes outward suggests a strong masseter.
Likely fat
Soft tissue you can pinch under the chin or along the jaw that does not change when you clench.
Likely laxity
Gently lifting the cheek skin upward sharpens the jaw edge noticeably.
Likely proportion
From the side, the chin sits well behind the lips, making the lower face look rounder.
If none of these change much and the width feels hard and fixed, bone structure may be the main factor. Bring your observations to the consultation; Dr. Kim will confirm them with a hands-on examination.

Compare
Jaw Botox vs contour injection vs HIFU lifting for a smaller face
These three are the core of most small face plans, and they are often confused because all three are marketed as 'face slimming'. They work on different layers, at different speeds, and last for different lengths of time. Comparing them side by side makes it easier to see why a combination is usually suggested.
More detail on each is on our pages for jawline Botox, face slimming injections, double chin reduction and HIFU facial lifting.
| Compare | Jaw Botox | Contour injection | HIFU / RF lifting |
|---|---|---|---|
| Layer treated | Masseter muscle | Local fat | Skin and support tissue |
| Best for | Square jaw from a strong muscle, clenching | Under-chin or jawline fullness | Early sagging, blurred jaw edge |
| Sessions | One, repeated every few months | Usually 2–4 or more | Often one, repeated about yearly |
| Typical duration | About 4–6 months | Treated fat cells do not return | About a year |

Everyday factors
Clenching, chewing and swelling: what else affects face width?
Some of the width people want to treat is influenced by habits and day-to-day changes rather than structure alone. Knowing about them helps you keep results and avoid treating something that is temporary.
- Clenching and grinding: a jaw muscle that works hard, especially at night, can stay bulky. Jaw Botox may also ease the tension; if you grind your teeth, a dentist may suggest a night guard as well.
- Heavy chewing: frequent gum chewing or very tough foods keep the masseter working; cutting back can help Botox results last.
- Fluid and puffiness: salty meals, alcohol, poor sleep and the menstrual cycle can make the face look fuller for a day or two. This is not fat, and it is not what contour treatments target.
- Weight change: treated fat cells do not return, but the remaining cells can enlarge with weight gain.
- Age and hormones: skin firmness and facial fat change with age and around menopause; see menopause care.
Timeline
Small face treatment results timeline
Because each component works at its own pace, a small face plan unfolds over weeks to months. The first visit often combines jaw Botox, a lifting treatment and a first contour injection. Slimming from Botox becomes visible over two to six weeks, contour injections continue every few weeks if needed, and lifting keeps building for several months. A review at about four weeks shows what has changed and what is still worth doing.
Aftercare
Small face treatment aftercare: do's and don'ts
Aftercare depends on which treatments you had, and you receive written instructions for your plan. Botox and lifting have very little downtime. Contour injections usually cause a few days of swelling and tenderness under the chin or along the jaw, so plan important events and photos with that in mind.
Do
- Stay upright for a few hours after jaw Botox
- Use a cool compress briefly for contour-injection swelling
- Choose softer foods for a day if your jaw feels tired
- Apply sunscreen daily after lifting
- Keep your review at about 4 weeks
- Call the clinic for worsening pain, fever or a change in your smile
Don't
- Rub or massage jaw Botox areas for the rest of the day
- Use saunas, alcohol or do hard exercise for 1–2 days
- Chew gum heavily while Botox takes effect
- Judge contour results before swelling settles
- Book facial massage or dental work soon after without asking
- Repeat Botox sooner than advised

Evidence
Does small face treatment work? What the research says
There is no single study of 'small face treatment', because it is a combination. The evidence is strongest for its two most studied parts: botulinum toxin in the jaw muscle, and injectable fat reduction under the chin. Lifting devices and threads have their own evidence, covered on their pages.

Choosing well
How to choose a small face clinic in Seoul
Small face packages are widely advertised in Gangnam. Use these questions at any consultation, including ours:
- Is the plan based on an examination? A fixed package for every face is a warning sign.
- Do they explain which layer causes the width? Muscle, fat, skin or bone, in plain language.
- Are products and doses named? Botox brand and units, injection products and amounts, in writing.
- Does a doctor perform the injections? Ask who will treat you on the day.
- Do they tell you what will not work? Honesty about bone width is a good sign.
- Is there a review? A check at about 4 weeks before adding more.
What affects small face treatment cost in Seoul?
The cost depends on which components you need, the Botox dose, the type and number of contour injection sessions, the lifting device and number of shots, and whether threads or filler are added. Cosmetic procedures are not covered by Korea's National Health Insurance. You receive a written quote at your consultation before you decide.

Korean small face treatment: frequently asked questions
Can non-surgical treatment really make my face smaller?
Non-surgical treatment can make the face look noticeably slimmer when width comes from jaw muscle, local fat or sagging tissue. It cannot reduce bone. The change is usually moderate and natural rather than dramatic, and the doctor will tell you at consultation which parts of your face are likely to respond.
Which small face treatment works fastest?
Energy lifting and threads give some visible tightening within days, while jaw Botox takes 2–6 weeks to slim the muscle. Contour injections work gradually over several weeks per session. For a short stay, a combination started on the first visit gives the most progress, with results continuing after you fly home.
How long does small face treatment last?
It depends on the component: jaw Botox usually lasts about 4–6 months, lifting about a year, and threads 6–18 months. Fat cells reduced by contour injections do not return, but weight gain can enlarge the cells that remain. Most people plan maintenance once or twice a year.
Is small face treatment safe?
When performed by a physician with appropriate products and doses, these treatments have well-understood safety profiles, but every treatment carries some risk. Bruising, swelling and temporary chewing fatigue are common. Rare complications include an uneven smile, nodules and, with filler, blocked blood vessels. The doctor reviews your medical history before planning treatment.
Can I combine several treatments in one visit?
Often, yes. Jaw Botox, energy lifting and a first contour injection are commonly done in the same visit. Threads and filler can sometimes be added, but some combinations are better spaced out. The doctor will advise the order and timing that suit your plan and schedule.
Can foreigners get small face treatment in Seoul?
Yes. International patients can have small face treatment at Hommage Clinic; bring your passport for registration. Tell us how long you will be in Seoul so the plan can fit your stay, since some components need repeat visits. Contact us before your visit to arrange language support.
How do I know if my face is wide from muscle or fat?
A simple clue is to place your fingers at the jaw angle and clench: a firm bulge that pushes outward suggests a strong masseter muscle. Soft tissue you can pinch under the chin or along the jaw that does not change when you clench suggests fat. The doctor confirms this with a hands-on examination at your consultation.
Does jaw Botox make the face smaller?
It can, when width comes from a large masseter muscle. A systematic review of 20 studies found that botulinum toxin narrowed lower-face width by reducing masseter volume, with repeat treatment needed to maintain the effect. It will not help if the width is mainly from bone or fat. See jawline Botox.
Can a double chin be treated as part of small face treatment?
Yes. Fullness under the chin is a common part of a small face plan and is treated with fat-reducing injections, sometimes with lifting. In a phase III trial of a deoxycholic acid injection, 66.5% of adults improved by at least one grade versus 22.2% with placebo. See double chin reduction.
Will my face look hollow after small face treatment?
It should not if the plan is conservative. Over-treating the jaw muscle or removing too much fat can make the cheeks look slightly hollow, especially in slim faces or with age. Your doctor balances slimming with support and may suggest lifting or a little filler instead of more reduction.
Can small face treatment replace facial contouring surgery?
Only when the width comes from soft tissue. Surgery such as mandibular angle reduction changes bone and gives a permanent structural result, with general anesthesia and weeks of recovery. Hommage Clinic does not perform bone surgery, and the doctor will tell you if your width is mainly from bone. See cheekbone reduction.
When can I fly after small face treatment?
After jaw Botox and lifting, most people can fly the next day. After contour injections or threads, allow a few days for swelling to settle, especially if you have photos or events when you land. Ask for a written record of the products and doses for any follow-up at home.
Is small face treatment covered by insurance in Korea?
No. Small face treatment is cosmetic and is not covered by Korea's National Health Insurance, and travel insurance rarely covers it. The cost depends on the treatments, doses and number of sessions, and you receive a written quote before treatment.
