What causes a double chin?
A double chin is fullness under the jawline caused by one or more of three things: extra fat beneath the chin (submental fat), loose skin and weakening support tissue, or the shape of your jaw and chin. Knowing which one dominates matters most, because each responds to a different treatment.
- Fat: a soft pad you can pinch between your fingers. It is often inherited and can appear even at a healthy weight.
- Laxity: skin that drapes or creases when you look down, often with a softened jaw angle. It increases with age, sun exposure and weight loss.
- Structure: a small or set-back chin makes the neck look shorter and fuller in profile. Adding projection with chin filler can improve the angle without removing anything.
Less often, neck muscle bands or enlarged salivary glands play a part; these do not respond to fat reduction.
Double chin treatment options: which fits your cause?
The rule of thumb is simple: reduce fat when fat is the problem, tighten when skin is the problem, and combine both when you have some of each.
If fat is the main cause
- Deoxycholic acid injections use a synthetic form of a bile acid your body makes to digest dietary fat. Injected into the fat pad, it breaks down fat cells, which the body clears over several weeks. Most people need 2–4 sessions a month or more apart. Many Korean “contour” injections are mixed formulas, so ask which product is used. Our fat-dissolving injections page explains the differences.
- Cryolipolysis cools the fat pad through a small applicator so that some fat cells die and are cleared gradually. It avoids needles but usually gives a more modest change per session. See cryolipolysis for how it works.
If loose skin is the main cause

If you have both
A common sequence is to reduce fat first and tighten afterward, so the skin can contract over a smaller volume. Removing fat from very lax skin alone can make the skin look looser.
What happens during double chin treatment?
Every plan starts with an assessment, and the treatment itself usually takes under an hour. Most people return to normal activities the same day.
- Assessment: the doctor examines your chin and neck upright and looking down, pinches the fat pad, checks skin elasticity and takes photos.
- Injections: the skin is cleansed and marked with a small grid, and ice or numbing cream is applied. Small amounts are placed into the fat only, keeping away from the jawline border where a facial nerve runs close to the surface. A burning or stinging feeling for several minutes is normal.
- HIFU or RF: after numbing cream, the device is passed over the area in lines or squares. You feel short pulses of deep heat, and the energy is adjusted to your comfort.
- Afterward: a cold pack is applied and you receive written aftercare.
How long does double chin reduction take to work, and do results last?
Results appear gradually rather than overnight. After fat-dissolving injections, swelling settles over 1–2 weeks and the slimming becomes visible about 4–6 weeks after each session. HIFU and RF tightening develop over 2–3 months as new collagen forms.
Fat cells destroyed by injections or cryolipolysis do not grow back, so the change is long-lasting if your weight stays stable, although weight gain can enlarge the remaining cells. Tightening typically lasts about a year, and many people repeat HIFU or RF yearly.
Aftercare: after injections, use a cold pack on the first day or two, sleep with your head slightly raised, and avoid saunas, alcohol and intense exercise for a few days. After HIFU or RF you can usually wash your face and wear makeup the same day.
Risks and possible side effects
Every double chin treatment carries some risk, and the type depends on the method. Your doctor explains the risks of your option before you consent.
- Fat-dissolving injections: marked swelling, bruising, tenderness, numbness and firm areas under the skin are common and usually settle within weeks. Less common risks include temporary weakness of the lower-lip muscle that makes your smile look uneven, temporary difficulty swallowing and, rarely, skin ulceration if product is placed too close to the surface.
- HIFU and RF: redness, swelling, tenderness and tingling for hours to days. Less often, bruising, temporary numbness, small burns or blisters.
- Cryolipolysis: numbness, redness and tenderness for days to weeks. Rarely, paradoxical adipose hyperplasia, where the treated fat becomes larger and firmer instead of shrinking.
- Threads: bruising, dimpling, a pulling feeling and, rarely, infection.
Seek urgent care if you have difficulty breathing or swallowing. Contact the clinic if you notice an uneven smile, skin that turns dark, blisters or breaks down, pain or redness that is increasing, a fever, or swelling that is still getting worse after the third day.
Non-surgical treatment vs liposuction or a neck lift
Non-surgical options suit mild to moderate fullness and laxity. If you have a large amount of neck fat, heavy sagging skin or prominent neck bands, surgery such as submental liposuction or a neck lift usually achieves more in a single step.
These operations are performed by plastic surgeons under sedation or general anesthesia, with 1–2 weeks of visible swelling and a compression garment. Most double chins are a mix, but one cause usually dominates. The profile view shows the difference best. A fat-type double chin forms a rounded pad between the chin and the neck. A laxity-type double chin forms folds or a soft curtain of skin. A structure-type double chin comes from a short or set-back chin, which shortens the line from chin to neck even when there is little fat.

When you have both fat and loose skin, the order matters. Reducing fat first lets the skin settle over a smaller volume; tightening afterward then works on what remains. Before your consultation, try these three simple checks in front of a mirror or with a side-view photo. They do not replace an examination, but they help you describe what bothers you and understand the plan you are offered.
- Pinch test Gently pinch under the chin. A thick, soft roll suggests fat; a thin layer of skin suggests laxity.

Does double chin reduction work? What the research says
Under-chin fat is one of the best-studied areas in aesthetic medicine because of the deoxycholic acid trials. In two identical placebo-controlled trials with more than 1,000 adults, about two in three people improved by at least one grade on doctor and patient scales, and side effects were mostly temporary. The evidence for tightening devices is more varied and harder to compare.

Is double chin reduction safe while breastfeeding?
Fat-dissolving injections and most energy-based treatments are postponed during pregnancy and breastfeeding, because their safety has not been established in these situations. Can double chin injections affect my smile?
Uncommonly. In the deoxycholic acid trials, about 4% of people had temporary weakness of the nerve that moves the lower lip, making the smile look uneven; all cases resolved on their own. Injecting only into the fat and staying away from the jaw border lowers this risk.
